	Every time this is applicable for each and every patient admission through your reference.
	Concession, if not claimed will be kept in sai shree hospital’s account in the form of points which can be claimed by any time in future.
	This scheme is applicable for IPD as well as OPD treatment.
	This scheme is going to be a boom for those who have not mediclaim/health insurance for themselves/for their family.
	No age limit for this scheme.

After registration management will issue a bar coded card with the name address and photograph of the registered person. This card is necessary for each transaction in future.
1. 10% of the bill (excluding cost of implant, medicine, investigations performed outside the hospital) of a patient will go to the account of the person who has referred him to SaiShree Hospital for treatment in terms of points. (Each point is equivalent to one INR). 2. When referred patient refers the other patient that 10% will go to first referrer and 4% to 2nd 3. The chain will go up to 3rd reference and points will be 10%, 4% & 2% of patients bill That is if • A refers B A will get 10% points from bill of patient B • B refers C A will get 4% & B will get 10% points from bill of C • C refers D A will get 2%, B will get 4% & C will get 10% points from bill of patient D 4. The points can be used to reduce the bill on OPD charges & investigations performed in hospital and for IPD patients also. 5. The points will be accumulated in two different heading one is points on consultation & second is points on hospital bills. These points can be used to reduce the bill in the respective headings only. 6. Points can be utilized to reduce the hospital bill excluding cost of medicine, implant and investigation performed outside the hospital. 7. There is a option to encash the points above limit of one lack at any given time. 8. When you go for encashment you must maintain 1 Lack points in the accounts. 9. Only the bills of Saishree hospital can be reduced and this service cannot be availed to reduce for bills of other hospitals. 10. Redemption of points to reduce the bill will depend on already accumulated points in the account of card holder as on date. The points from present bill cannot be considered for redemption. 11. The points are not transferable, it can be redeem against the services of SaiShree Hospital only. 12. All the information is compulsory to feel in the form. 13. Redemption of points can be possible for Self or any other Sanjivanee scheme members; this facility is not available for the non members. 14. If card holder is minor (Less than 17 Yrs.), His guardians have rights to utilize the points. 15. Duplicate bar coded card will be issued at the cost of Rs.250/- 16. No bonus points will be issued for the patients enjoying Cashless facility during the hospital stay or services based on OPD basis. 17. The insured patient can utilized these points for his OPD or IPD treatment which not covered by Insurance company. 18. Management reserves the rights to change the rules & terms and conditions without any prior notice. * The points will be given on bill, excluding the cost of medicines, implant and the cost of Services & investigations required from outside the Saishree hospital.
